ER Diaries : The P,H you should never ignore

Hello people

Today is a " Gyaan " day.

One thing per day . You learn everyday.
There was one fine day , when the emergency room was dead busy. I had just intubated a case , some more critical cases to deal with , discharging few cases. In short , perfect fish market kind of a situation . My incharge asking me status of every case . Just then a lady walks by , a priority 3 case attendant. She enquires about the lunch...

When you are at the end of your shift , sorting things out , hungry . Somebody asking you about it. I was too busy to care , I directed her to the nursing staff. Before she could go , my senior asked her to stop.

Madam , lunch? I'll order that for you. What would you like to have ? Let me check with the food and beverage department what they have in store. They have so and so for lunch . You'll have this . alright. Spicy or bland ? Haan ji . Please wait . Theyll get back to you .

I stared in disbelief. Are you kidding me? If it was sarcasm that was on fleek. If it was not . What was it?

As soon as she left. He smiled at me . And the gyaan started.
 " Patient ke pet aur peet par laat nahi maarna chahiye!"

Wait , whom did I hit and where?

I stood there with a weird expression.

I know you're busy . But there are certain things that should be addressed first . When people walk out of ER , they won't appreciate the milestones you achieved like activating stroke or sepsis first.Your timely intervention and diagnosis will all remain unappreciated . They would go back and complain about how long they were out of their comfort zone. Like making trips to various departments, billing etc . Hunger and Pain management have to be done quickly. I don't want your priorities to change. Priority 1 will be priority 1 .

My take home points were :
A hungry patient is an angry patient.
You don't forgive the people who hurt you and those who underestimate your pain.

So, the P is pain , H is hunger . address them SOS.

  "Experience-Based Co-Design" we have all witnessed its essence unknowingly . My family home is the example I can reflect on .Growing up I didn't like how my house was designed. Parallel Universe : BCT Series XXX

  Two moments in the ED this week made me realize that I come from a parallel universe. One involved a teenage patient who had applied bronzer that I found while cleaning her skin for a IV cannula which left me surprised as I come from a part of the world where teenagers paint their skin with whitening agents to look more attractive. The second moment was during my teaching session on Organophosphorous poisoning, where I wondered if my accent was causing disengagement . However, I learned that it was simply a rare presentation of organophosphorous poisoning in this part of the world. It felt like I was narrating my adventures from a tropical trek! Have you ever had a moment where your cultural background clashed with your environment ?

Hello everybody  I hope everybody is safe and healthy. it's been a while that I've been receiving lot of patients in the emergency with mental illness ,thought of sharing an incident of its kind. Not sure if its the awareness of mental health these days or the pandemic, I am developing keen interest in patients with mental illness .I try to take an extra effort to make them feel comfortable and try to create an insight of what is happening and offer them options of what all can be done.  Since this it seems like a step towards patient care , I thought I'd share it here as well. This would probably not be the best approach but I welcome any type of constructive criticism , additions or subtraction to my approach because end of the day we are here for best patient care.
