I am back this month with an exciting incident to share!
Midnight on a night shift I receive a call from my triage doctor stating he has a lady whos complaining of bloating, he has asked about Last Menstrual Period and they're not okay revealing it. I asked him to hold the patient there while I come to examine. Next moment, was my triage doctor inside with the lady on the wheelchair right behind him.
The first visual was her expression of pain I saw through the counter. I asked them to take her on a bed while I rushed to see her. My teammate told me something is fishy. I see a gravid uterus not less than 30 weeks ! Don't tell my husband! I hid it all this while. Please , please! She said. I stared in disbelief. I asked her about how many weeks , she told me around 25 weeks. I ran to get my sterile gloves. My nursing staff came to me and said mam! Come fast ! She seems to be in labour. When I examined I could feel the head and the membranes were about to rupture.
Okay! Now after 2017 I have never attended any lady in labour. I tried to get the best recall of the steps to be followed. The Obstetrician on call was 30 minutes away. Pediatrician was not reachable.We had two choices either to deliver it there or send it to the nearest obstetrics centre. The latter was no more an option in next few minutes.
Head unable to pass out of Os downward abdomen push applied , my reflexes caught hold of the scissors went ahead with episiotomy to facilitate the labour.
Emergency delivery of a male baby at 12:20am .Delivered the placenta too and then sutured the perineum area. Meanwhile my EM consultant wrapped the baby in dry sheets /warm blanket and cord clamped 2 minutes after delivery.Baby had good strong cry.
on call obstetrics then arrived. Both mother and baby were observed for sometime and transferred to other center.
The background story was she had 2 daughters previously, the husband wanted a male baby. She was just too scared to reveal she was pregnant again, she missed all antenatal checks and vaccinations.
